New Health Programs for Seniors |

Program Reaches out to Seniors La Plata County Senior Services and San Juan Basin Health Department, through a Colorado Trust grant, provide a Senior Outreach Nurse to assist all community members aged 65 and older in La Plata County. The nurse assesses an individual’s health needs and provides referrals to various community and health related programs that can assist the individual in enhancing their health and quality of life.
Since the program began in September 2007, Senior Outreach nurses Tracy Davis and Brenda Isgar have already seen about 400 clients. Referrals to the program can come from anyone, including a family member, physician, pharmacist. The visit is free. New Program Provides Health Screenings for Adults A new program at San Juan Basin Health Department seeks to reduce risk of cardio-vascular disease among adults in La Plata County. The program will provide free health screenings, risk assessment, education and referral. Health screenings include blood pressure checks, blood glucose and cholesterol tests as well as education about risk factors. From these screenings, the outreach worker will identify people who are at moderate or high risk and provide referrals for follow-up. Funded by the Community Cardiovascular Disease Prevention Program, this one year grant provides funding for a full-time bilingual community health worker who will be screening at sites throughout La Plata County. The goal is to reach at least 600 uninsured or under-insured individuals during the next year. Rotary Cares project Helps Support Living Independently San Juan Basin Health would like to recognize the local Rotary which has recently stepped up to the plate to help fill gaps that currently exist in supporting the elderly to continue living independently in their own homes. The “Rotary Cares” project provides not only funds, but manpower as well. |
